I made a form for my site http://recorderman.com/index.php?option=com_chronocontact&Itemid=68 everything seems to work but when form is sent to me some of the fields are missing text see below
the school name address and phone number aren't coming through can someone help me figure out the problem please

Hi budinakron ,

School Phone Number has spaces in the input name which aren’t allowed, plese remove them e.g. school_phone_number if OK.

The other two look OK in the form; please check that the names there match the names in the Email template exactly.

Bob I tried that and still not showing up here is the email template. any suggestions? I can give you admin access if you can look at it and tell me what I did wrong

Well, you need to update the Email template so that the name match exactly.
