Form Won't Send Email etc.....

I have a form here: http://www.silkwebsolutions.com/elliptic/en/products-a-solutions/free-evaluation and I cannot get the email to send to the email address with the data from the form as well I cannot get the form to return onto the page after the form is submitted.

I am using "static" email and have it set to myself and no matter what email address I use to test the form the "thankyou email" gets sent to my email address rather than the data from the form??

HELP?

Also, I read that if you want 2 forms on the same page that the fieldnames have to be different so I changed all of the field names and now the second form doesn't appear at all and I didn't change the form name.

HELP Please?

I don't see any form on the page you have linked to :-(

If you have two forms on the same page then the input names and ids should be different to avoid problems with validation. It shouldn't affect the form's visibiity unless a typo crept in when you made the changes.
