Can't seem to receive the form field data in email?

Hi. I have a very simple callback form. When i fill out the details i receive an email but the only information i receive is "You may customize this message under the "Template" tab in the Email settings box." I do not get any of the form field data. Any ideas what to try?

Do i have to fill out dynamic fields in the email configuration?

One more thing while im here....
Years ago i purchased a license for chronoforms. Is it still valid and if so where can i find it?

You have to enter some email body text in the template tab - just next to the static tab.
