Strange issue on submit

There is a strange error? on submitting a form for submission. It is a simple form that sends to email using recaptcha. Worked for quite a while and I was informed by a user that when he submits he gets a pink bar. I went and tried and got the same. Attached is a screen capture of that

The user is using IE8 and I am on IE9. Any ideas what it might be or what I can try?

Is this ChronoForms version 3.2?

If so there's a bug in the File upload code. Please see this post.
