Table Creation missing all fields from Wizard Form

I Created a form with the Wizard,and then did Create Table, and the database table has no fields.

When I did this for an imported form, it worked perfectly, all fields created.

I am attaching a backup.

Looks OK here. I restored the form and went to Create Table. You do have to click the icons at the right to turn the bars green and select them. Or the (badly imaged) icon marked below to select them all at once.

Thanks for that. I got the error from Create Table that the table already existed, which it did.

I created the table in PHPMyAdmin with the provided code, but could not get data to add.

Renamed the Form, and started fresh with Create Table, etc. and now data does get added.
