2 problems here:

1. the formatting is not correct for my check boxes - how to fix?

2. the email results do not reflect results when boxes ARE checked...

Hi LrG,
This is an issue with the Form Styling feature of your RocketTheme Template. Disable this, and you'll see everything working as it should.
