We had our website transferred over to a new host and I noticed a couple things broke.

Second: I disabled the validation and filled out a test application. the application sent successfully but NO ONE recieved the forwarding emails. this all worked fine before the transfer, could the issue be related to the same problem?

Do you mean the imageverification or the input validation?
Imageverification is probably either permission problems or that the PHP GD library isn't installed. Your ISP should be able to help.
Emails probably mean that the site mail setup isn't correct in the Global Configuration.

The problem were having right now.. my host said I need this.
When I contacted the previous site owner he said this..
Is there no way to get that back? Will re-installing the plugin work?

We are missing one more database: shadowr4_rnd01

The databases...unfortunately for this case, ALL of them were deleted the same day the site was verified to be successfully transferred. You'll need to have a new one created by deleting the chronoforms plugin and re-downloading it and installing it. I believe it'll make a new one.

I've no idea what the 'shadowr4_rnd01' database /table is. Not anything that ChronoForms creates by default.
Installing ChronoForms will create the basic tables that you need but they will be empty; it can't recreate the data.
Did you not have a full db backup to move the site?
