Using dynamic elements without wizard

Hello all

I am using form HTML that I've already created, and I want to use form results in the dynamic Subject, From Email, Reply To fields. I can't seem to figure out how to do that, and couldn't seem to find the answer in the forum.

Just use the 'name' of the form input in the Dynamic boxes - no quotes and no brackets.
