Form Duplication

I have created a Form but it is appearing twice upon publication.
I have tweaked all the settings I can think of to try and stop this occurring but
for some reason the form is still appearing twice on the page.
When I hit the submit button one of the forms disappears but the other remains.

Has anyone any ideas on how this might be resolved,

It looks as though your template may be posting the contents twice? is that possible?

thanks for coming back to me. I've tried some of the other templates but the duplication is occurring with them also, so that doesnt seem to be the answer,

I solved the problem. The problem was occurring in Module Manager. The ChronoForms module (mod_chronocontact) was assigned to all the pages available under Menu Assignment/Menu Selection. Once I changed this from All to None the duplicate entry disappeared - leaving me with just the one form as required.
