RC5 - J1.5.15 - date picker doesn't work

Hi, I just installed the latest of everything on this site and created a form which can be viewed here: http://69.16.221.236/index.php?option=com_chronocontact&chronoformname=item_submit. The "date Picker" just won't work. I've redone the form 3 times thinking it might be me - when I am creating the form using the wizard the datepicker seems to work during creation but once published it does not. Any help?

Hi sitebuildernow,

It looks as though there is a conflict with something in the template utilities. If you view the form without the template the date picker is fine.

Unfortunately I'm not seeing any error report than might give me a clue where the conflict is.

Yeah, I had hoped that it was a js conflict - but I only have a couple of plugins installed and I deactivated those and it still doesn't work - I also tried switching templates - no luck. Any other ideas? Thanks for any help you can give.
