You can insert images in Joomla using an extended editor plugin Joomla! Article Image button.
well yes they have to, first of all, all images should be 72dpi for start. then it depends on what size you want it to be 200 x 300 px or whatever you need it to be. you should resize just for the fact of reducing file size. now regarding positioning maybe learn some css that should help if its more complex. but even a good editor can give you a few options if you dont want to learn css.
You may try to install some plugins, such as phoca gallery to insert images in Joomla content.
I would be interested in learning the answer to #4, how to position your images. I find that sometimes the images in my site tend to be off center and the text does not wrap and it messes up the overall uniformity of the page. How all the images to stick where you want them?

I have used Joomla's Media Manager to upload the images, but the main thing is you have to place the images in images/stories.
You can use Simple Image Holder to insert and position images. It's highly customizable, XHTML compliant, and totally free.
What is {mosimage}?
It is a simple way of including images in your content.
Just add some picture (upload the images using Joomla's Media manager), then insert {_mosimage_}(without the underlines)in the place you want an image to appear and save your content item.

A simple screen capture or how to use one of the plugins would be even better.
