can't link to first form

I have Joomla 1.5 and Chronoforms 3. I have ensured that Chronoforms is enabled in the Module manager and that chronocontact (which I think is the Mambot) is enabled in the Plugin Manager. I have created a simple form and table and menu via the Chronoforms Component and made sure they are Published.

I have added the code to an existing Front Page as per bellow:

The {chronocontact]Feedback{/chronocontact} will be replaced by the complete Form HTML - not by the url to link to the form. The url will look like 'index.php?option=com_chronocontact&chronoformname=Feedback'
