Checkbox problems (again)

I am using chronoforms to get infos on user before they can download files.
I have 4 textboxes and 1 checkbox.
Everything is going smoothly but I do'nt receive any infos from that checkbox when I check my records. I have my 4textboxes recorded but no sign of my checkbox.

Any idea ?

received them where ? in the Email or the database ? how did you create your form ? show me the form code in the form HTML code box and email template code as well!
