Made the tiniest of changes and form no longer appears.

Hi All,

I made a change to a form to amend an email address as form submission goes too and ever since then the form no longer even appears on the live published site.

It all appears fine in the Pages Preview with all questions and everything still existing but when Previewing it elsewhere the form is just blank with just a link to this site (which used to sit below the form)

Also, Is there a way to change back to Easy Mode after swapping over to 6.1 as I can no longer find any dropdown to change designer mode like I could in 6.
