The page can’t be found.

When I try to view the frontend version of my form I get the error "The page can’t be found. It looks like nothing was found at this location.". I can see my form in admin mode. Any ideas on hope I can resolve this? I have some images I could attach if that was an option.

and when you load the form inside a page/post using the shortcode ?

When I insert the shortcode [selah_camp_2023] on a page it just shows the shortcode [selah_camp_2023]. I'm new to shortcodes so I may be doing something wrong.
