How to add a form to component pages?

I need to be able to replace the current form on our vehicle offer pages similar to the one at the URL above. We have around 400 offers so I need the form to appear on all pages and for the subject field to be populated with the vehicle make and model as it currently does.

Is this possible and if so, is it fairly straightforward for a newbie like me.

All i really need to do is add a couple of additional form fields. I would use the existing form that comes with the component we are using although adding a couple of fields to this doesn't seem straightforward either.

Back to the drawing board I guess .... unless you have any suggestions??

Hi pj,

even showing the form there after a non straight forward steps will not be the end of it, the form action url will need to change and other data need to be loaded, a punch of things! not easy at all! when I need something like this I just create a complete component from scratch or use a customized Chronoforms form!!😀
