Form not display in popup

I have 2 forms on site.

When I'll click on the button, form doesn't display in popup.

Have you tried clearing your browser cache to see if you have some old code there?

AJAX is enable. I see how in this modal window website loading...

From the image it looks as if you are re-loading a whole page in the pop-up.

Are you certain that you have Ajax submit enabled?

Do you have a Redirect action in your form On Submit event?
