This error only occurs in Easy Mode. To replicate the error:
1. Create a new form with Easy Mode designer.
2. Add a Text field. (field_text0 appears)
3. Click Apply or Save.
4. Open the form again, and add a Text Field.
5. The new Text Field will have the same incremental number (field_text0). If you then save the form it removes one of the Text fields.
Same thing happens with Header field.
