form doesn't work

resolve a ChronoForms form not displaying on a website.

Overview

The form was not visible because it was either unpublished or had an incorrect name.
Ensure the form is published and that the form name in the CF settings matches the name used in the menu or module link.

Hi leadersoft,

Please check that your site can send emails using the Test Email link in the Site Admin > Email settings area.

On the form "contact-us" which already integrated on the template I can not see the fields on the site :

Hi leadersoft,

What is the form name? If I try to load http://www.ths-medicale.com/index.php?option=com_chronoforms6&chronoform=contact-us I get "Error, form does not exist or is not published."
