I did include 2 forms on 1 page. First form working correctly, but second form not.
After click submit button on second form, this form refresh and display first form already.
Where is problem?

How did you load the 2 forms on the same page ? using modules or plugins ? do the forms have unique aliases ?
Please try to open the display section action settings in each form and set the "Action url" to
