Or better to make a second container appeared after the start writing the first Container? Submit button would be the first of container.

It looks as though you are using the Ajax submit but your server is refusing the connection:

I suspect that this is because the form Action URL is invalid.

Does the form work from the View Form or Test Form links in the Forms Manager?
