Form does not submit, no emails, no thanks message

My site is htsgteam.com. I have used Chronoforms several times already on this site and they are working well. However, I ahve added another form and I can't get it to work as it should. It does not submit, there is no email sent when the form is submitted, and there is no thanks message shown when the form is submitted. On submitting, I am sent to a blank page, with no thanks message. Here is a link to my form:

I can't figure out why it will not work. I am a subscriber and I love your forms when they work, but this one is kicking my butt. It is a rather long form, but that should not be a problem. I have used the custom code option to develop this form, but most of the code has been copied from another form.

As far as I can tell from the front-end the form submits correctly. What do you have in the On Submit event?

Attached is a screen shot of my setup. I have "Load Captcha" in the OnSubmit event.

I am also not getting anything from the "Display Message event.

The Event Loop action redirects the user back to the form On Load event so nothing after that will be run i.e. all that happened is that the form is re-loaded.

Please re-create the Display Message and Email actions after the Check Captcha action.
