Do you have the "Attach file" setting enabled in the file field settings ? and do you have the "Auto attach files" setting enabled in the email action ?

Did you try ChronoMyAdmin for managing your Joomla database tables ?
I have "Attach to email" in file upload field setted to ON and also "Auto Attach file field" setted to ON in the email action but no file is attached to the email but correctly uploaded to the server folder.
Could you please upload the form backup some where so I can test it ?

It was a size limit problem. If we upload two files and attach them to the email, and one is bigger than the size limit, only one is uploaded (the smaller) and none of them is attached to the email.
