I want to bring all this into Joomla and ChronoForms/Connect

What I need is a quote to ...
- rebuild the form so that it loads up with data from a MySQL table called "Allocated" that I will publish INSIDE the Joomla Database.
- have the form publish results to a new table called "JobReports" inside the Joomla Database.

There are some requirements that I will need to explain to you if you are capable and interested in the above job.

This is an Urgent Requirement.
