Reload text field before send. Please Help!

Hello!
1. In form "A" fill the fields, dato1, dato2, dato3, dato4...
2. If dato5 is not found in dropdown click the button to open popup...
3. fill field of popup form and register...
4. The form B is close automatically
I lose dato1, dato2, dato3.... ¿How can i reload it?.

do not open a new form, add "register new" to the dropdown (or keep the button) and when you click it, show a hidden area with the field you have!
