hello every one

trying to do a survey system for companies about internship program
​
saying each company has 1 to 5 internship
i need to measure the feedback with
radio buttons 1 to 5 points
​
for example if a company has 1 internship it will register that student on db thinking insert php code
​
if the company has three internship three times companies need to do the survey so i can have three new registration on the table
​
i am not clear about how to do it
trying multi page
but not necessary i suppose
