File upload / Attachment issue

Hello

File attachment is not working in the email.
all information get into the email except attachment file.

Please resolve asap.

Chronoforms Version: 5
PHP Version:5.6.38
Joomla Version: 2.5.28

Hi directivemsp ,

I restored your form replaced the To Email and the Google codes with my own and the form works correctly including the file attachment.
