Issue with public users

Hello All,

I have created form that works perfectly fine for the admin. The problem that I encounter is when public users are trying to submit the form, they dont get calendar picker and not able to pick anything from the dropdown.

Hi varlamov,

You have some JavaScript errors on the page that need to be fixed - you also have some odd and invalid CSS in the page <body> tag.
