Stay at the same page after popup form

Hi,

May I know how to stay at the page after the popup form displayed? As the picture shows below, the page is gone at the background after the popup form displayed.

There is no way to tell from the information here. Maybe something wrong with the popup code, maybe a JavaScript error, maybe a PHP error???
