Contact Form, Chronoforms 5, Fields Won't Validate, load state disabled does not work, Required * not displayed

I am using Chronoforms 5.0.12, joomla 3.7.5, PHP 5.4.40, JoomShaper template. I have a basic contact us form, I have a required field, named "Question", set to "yes". I even tested the "demo-basic-contact" form and the validation won't work.

Also, the question field is a text area and I set it to 12 rows, 80 columns and it just shows a couple rows. Also, the "*" that shows that it is a required field in the label is not displayed.

Also, I have two fields, name and email address that are self-populated and have load state = disabled so that users cannot edit the fields. This does not work, a user can edit the fields.

If I press the submit button, it does send an email with the formatted text in the email body to the correct person. It does basically work but the validation, not allow editing in certain fields, required label * are not working.

On the same server, I have the same contact form in another site using Rockettheme template with chronoforms 5.0.12, joomla 3.6.5, with PHP 5.4.40 and the contact us form validates, "*" shows in the field label to show it is required, and the name and email address fields do not allow users to edit fields since load state = disabled.

My server is in a company intranet with no access from the internet.

Any suggestions or work-arounds? There are no error messages in the firebug console. I cannot switch to the Rockettheme template since I have completed development and want to deploy this site ASAP. I cannot switch to Chronoforms 6 since the PHP version is too old - PHP 5.4.40. We will be upgrading to PHP 7 within the next 6 months but not now.

Sorry, I cannot post a link because the form is on a server in my company's intranet with no outside access from the internet.

Hi golfgal,

Hard to tell - it sounds to me as though some external files are not being loaded - possibly the font for the required * and maybe some or all of the jQuery files that enable the validation etc.

If you want to attach a copy of the form backup (use the Private tags if you like) then I can check to see if the form works on my test server.
