Let the front-end author edit a form ?

Hi

I have successfully created a multilingual form on the website for my sports club:

http://www.elhatri.nl/index.php/general/gratis-proefles.html?lang=en

Now I would to add the possibility for the owner of the club to edit the form on the front-end. Is this possible and how ?

I have already searched the forums but the solution isn't there or it's to hard for me to understand.

If it can be done with php/mysql then I would like to learn it. Have have no experience with that but anything to help me in a good direction, maybe a good starters tutorial would be great!
