Chronoforms v5: Sublabel not shown

Concerning: http://www.tuinhuizenfabrikant.be/en/catalogue-request

Altough I've filled in the sublabel for the Telephone field, it is not shown in the frontend.
I've also tried with a placeholder, but also not shown.
Can u help ?

[note: you have to check any "country code" before Telephone becomes visible].

The link to the form does not work!

Maybe the sublabel is hidden with some CSS code in your template ?

Did you try ChronoMyAdmin for managing your Joomla database tables ?

Hi FRank,

I can see the form OK - but there is no sub-label in the HTML for any of the phone numbers. Please take a Form Backup using the icon in the Forms Manager and post it here and I'll take a closer look.
