New form with multiple options - generate xml doc.

I'm looking for someone to create a form for our website where the user registers, then selects from 2 choices ie. dog or cat. The user can then add additional members ie 2 dogs and 1 cat etc. At the end the user has a discount applied for multiple members and an xml form is sent to the database.
