Error with form in article using cb fields

Form opens from backend:
http://www.golfclubwelschap.nl/index.php?option=com_chronocontact&chronoformname=zomer

But when included in article I get this error message:

Fatal error: main() [function.require]: Failed opening required '' (include_path='.:/usr/share/php4:/usr/share/php:/usr/lib/php/Smarty') in /home/sites/site353/web/components/com_chronocontact/plugins/cf_profile.php on line 4

Forms without cb_fields open ok:

Hi Astrid,

I don't know what's happening here but the simple answer is not to try to use the CB Registration PlugIn with the ChronoForms mambot - there are just two many different chunks of code trying to work with each other.
