Hi,

We are transfering our existing static site to Joomla. We currently have a Brochure Request form on the static site that serves us very well, and I'd like to have it tranfered to our new Joomla site.

Essentially, page 2 and 3 provide us with additional information if the prospect wants to provide it.

In addition to the details posted above, following is a "wish-list" for the project.

1. I'd like to send the brochure-requester a confirmation email that is based on the selections they made on the form. For example, if they indicated they are interested in diabetes, I'd like to send them a diabeties specific auto-responder email providing basic info until they can be contacted by one of our sales reps.

2. Also since our product is high-end, people often visit the site over a period of months and request brochures. Once this info is downloaded, it clogs our Access database with multiple entries for the same person. I'd like a unique identifer like email address that would let them update their existing info and request a new brochure or other material rather than creating a new entry.

As far as a timeline, I'd like to have this in place by 4/18/2008.

Finally, I work with a fabulous designer who can provide the styling and css. I'm primarily interested in functionality.
