Hi,

I've just posted a very simple sample form in the Downloads area here. It's a cfbak file that can be added into ChronoForm Form Manager using the Restore button.

We plan to add a few more forms in the next week or two; you can use these to see how ChronoForms works and as a basis for your own forms.
