I need a questionnaire form with tick boxes 2 Months, 2 Weeks ago Karma: 0  
Hi everyone

First off great component ive just installed but due to time constraints Im going to need some paid help.

I need to set up a health questionnaire with contact details then a list of questions with tick boxes (yes or no) with a couple of please describe: and a text box with the customer to add some text.

If anyone can help please let me know time is abit short on this one!

Hi peppercat,

Please send a copy of the form to the address in my sig. And please say what you want to do with the form results . . . emails, database adds, etc.

so the license is very simply for the same ChronoForms component without a link, thats all!

This License is for 5 different websites ONLY. 

 

 However, in order to allow everybody to still use the component and even get out of this, the link is inside a div with class : chronoform , use this to hide the link by using different colors or whatever if you really can't pay, but of course the link is still exists at your page source.
