Sample Form with Email Template

I don't normally do this, but I figured I would try and contribute to the scene...

I've created a sample form and an email template. The form is a quote request and feel free to modify it to your needs. I created the form and the email template in Dreamweaver. The template is basic as well as the form. Although I wanted to create something that utilized list menus, radio buttons, and check boxes.

As far as getting this up and running should be fairly simple. On the general tab make sure that you have the format to send results as "My Template". Under the "Form Code" tab add the code for the form under "Form HTML" and the email code under "Email Template".

As far as making any changes the only changes that I made were on the general tab. I left all the other tabs as they were. I turned on "Anti Spam" and set up "Validation" to run with the form. On the sampleform file I already have the {imageverification} included so if you want this to work make sure to turn on "Anti Spam".

Again, I don't normally do this. So sorry for the poor grammar.

Couldn´t figure out some of the email doodaah but with your example it was easy.

BTW, I found the easiest way for me was to copy the form in Dreamweaver, save as HTML email template, get rid of all the form elements and add the id´s for the fields. Then I added the resulting code as email template and voila, the email looks just like the form.
